Quick Checklist (Bring to the Airport)
Required Documentation & Gear (must bring)
- Physical ID Card (animal + handler identification).
- Recognition Gear: Service Animal vest, bandana, or harness.
- DOT Service Animal Air Transportation Form.
- Vaccination Records including rabies certificate.
- Microchip matching all documents.
- Veterinary Health Certificate (international routes).
Preferred Documentation (greatly facilitates acceptance and pre-clearance)
- Registration Certificate (our registry is accepted by all airlines).
- Prescription Letter confirming PSA or Service Animal need.
- Behavior Certificate.
Steps to Take (in Order)
- After booking, notify United Airlines of your Service Dog or PSA.
- Upload required DOT forms through United’s accessibility portal.
- Bring printed and digital copies to the airport.
Route Limitations (Service Dogs)
- Service Dogs and PSAs accepted on all U.S. domestic routes.
- International routes require additional import/export paperwork.
- ESAs are not recognized as assistance animals and must travel as pets if permitted.
