Quick Checklist (Bring to the Airport)

Required Documentation & Gear (must bring)

  • Physical ID Card (animal + handler identification).
  • Recognition Gear: Service Animal vest, bandana, or harness.
  • DOT Service Animal Air Transportation Form.
  • Vaccination Records including rabies certificate.
  • Microchip matching all documents.
  • Veterinary Health Certificate (international routes).

Preferred Documentation (greatly facilitates acceptance and pre-clearance)

  • Registration Certificate (our registry is accepted by all airlines).
  • Prescription Letter confirming PSA or Service Animal need.
  • Behavior Certificate.

Steps to Take (in Order)

  • After booking, notify United Airlines of your Service Dog or PSA.
  • Upload required DOT forms through United’s accessibility portal.
  • Bring printed and digital copies to the airport.

Route Limitations (Service Dogs)

  • Service Dogs and PSAs accepted on all U.S. domestic routes.
  • International routes require additional import/export paperwork.
  • ESAs are not recognized as assistance animals and must travel as pets if permitted.
