Quick Checklist (Bring to the Airport)
Required Documentation & Gear (must bring)
- Physical ID Card (animal & handler identification).
- Recognition Gear: Service Dog vest, bandana, or harness.
- U.S. DOT Service Animal Air Transportation Form (mandatory).
- U.S. DOT Relief Attestation Form for flights over 8 hours.
- Vaccination / Health Records as required by destination.
- Behavior Requirements: dog must be trained, calm, and under control at all times.
Preferred Documentation (greatly facilitates acceptance and pre-clearance)
- Registration Certificate (our approved registry is accepted by all airlines).
- Prescription Letter confirming PSA or Service Dog status.
- Behavior Certificate confirming stable, public-safe behavior.
Steps to Take (in Order)
- Complete and submit the U.S. DOT Service Animal Forms after booking.
- Upload documents to Delta’s online service-animal portal.
- Bring printed and digital copies of all documentation to the airport.
Route Limitations (Assistance Animals)
- PSAs and Service Dogs are accepted on all Delta flights nationwide.
- ESAs are not accepted under any circumstances.
- Some international destinations require additional documentation (e.g., EU, U.K., Japan).
- Dogs must remain on the handler’s lap or on the floor space without blocking aisles.
- Only two Service Dogs per passenger are allowed.
