Quick Checklist (Bring to the Airport)

Required Documentation & Gear (must bring)

  • Physical ID Card (animal & handler identification).
  • Recognition Gear: Service Animal vest, bandana, or harness.
  • U.S. DOT Service Animal Air Transportation Form (mandatory for PSAs & Service Dogs).
  • Behavior Attestation confirming the dog is trained to behave safely in public.
  • Veterinary Health Certificate depending on state or destination requirements.
  • Rabies Vaccination Certificate valid for the full travel period.

Preferred Documentation (greatly facilitates acceptance and pre-clearance)

  • Registration Certificate (our approved registry is accepted by all airlines).
  • Prescription Letter confirming PSA or Service Dog need.
  • Behavior Certificate confirming calm temperament and public-safe behavior.

Steps to Take (in Order)

  • Submit the DOT Form immediately after booking.
  • Prepare all veterinary documentation required for interstate or international travel.
  • Check in early and present documents at the counter and the gate.

Route Limitations (Assistance Animals)

  • Service Dogs and PSAs are accepted on all Alaska Airlines routes.
  • ESAs are not permitted in the cabin under any circumstances.
  • Some international destinations (e.g., Hawaii, Mexico) require additional health documents.
  • Cabin space limitations may apply for large Service Dogs.
  • Behavior requirements are strictly enforced during boarding and in-flight.
